Yeah, you’ll need to reinstall the game through Steam, since Steam also handles the patches.
But to actually find servers nowadays, you’ll have to install CoD4X it’s easy to set up.
Install Call of Duty 4 via Steam and make sure it is updated to version 1.7
Run the game once in multiplayer to initialize the files
Download the CoD4x client from the official release page https://cod4x.ovh/t/cod4x-releases/24

Extract the zip file you downloaded
Open your Call of Duty 4 Modern Warfare installation folder usually in
C:\Program Files (x86)\Steam\steamapps\common\Call of Duty 4 Modern Warfare
Copy the extracted files into the game folder and run the installer file to patch the game
Allow overwrite if asked
Start Call of Duty 4 Multiplayer from Steam
On the bottom right of the main menu you should now see CoD4x with the version number
In the server browser set the source to Internet CoD4x or connect directly with the console
